Nomination Form
The purpose of the Awards is to pay tribute to those who have overcome overwhelming challenges. These individuals deserved to be recognized and to celebrate their contributions to our communities. This recognition goes along way to encouraging these individuals to continue their efforts as role models for others.
The deadline to submit Nominations is January 17, 2025
Thank you for choosing to nominate someone who inspires you for a 27th Courage To Come Back Award presented by Wheaton Precious Metals and hosted by Coast Mental Health Foundation. We are so excited to review the hundreds of nominations we receive and celebrate the stories of triumph over adversity.
The Courage To Come Back Awards are given in four categories:
- Mental Health: A person who has demonstrated inspirational achievements overcoming the challenges of living with a mental illness, and who has given back to their community.
- Addiction: A person who has demonstrated inspirational achievements overcoming the challenges of addiction and who has given back to their community. Sobriety must have been maintained for at least the past five (5) years.
- Medical: A person who has demonstrated inspirational achievements overcoming the challenges of a medical condition, either from birth or developed in their lifetime, or following major trauma or injury which has required extensive physical rehabilitation, and who has given back to their community.
- Young Adult: A young person, between 19 – 25 years old, who has demonstrated inspirational achievements overcoming mental illness, injury, or addiction and who has given back to their community.
* Before nominating someone, please ask for their permission, and ensure they are ready to do so. Re-sharing a traumatic story and/or life experience can be extremely difficult and, if they aren’t selected, could become another negative experience. It’s important that they are aware of the process before you start.*
Award recipients will be given the opportunity share their story via video, with media and on social media. We follow trauma-informed storytelling practices and do our utmost to avoid harm during the process.
